The shared phone field represents a total purchase amount added once to each option. If the options use materially different devices, incorporate the difference into their respective setup fields and document that adjustment.

Make administration visible
Include patching, support, backups, certificate work and incident response in the hours assumption. If a support contract covers some tasks, account for its cost and avoid counting the same work twice. Free software does not remove the need for someone to operate it.
Ask whether the hosted seat price includes the calling, support and features being compared. Add missing recurring charges into your chosen cost inputs with a clear note.
